NCHC programs and projects
North Country Health Information Network (NCHIN)
- an intranet for communication and patient care coordination among member agencies;
- Internet access and web page services for member and non-member health agencies;
- access to health science library and online biomedical resources, and
- videoconferencing.
Northern New Hampshire Area Health Education Center (AHEC)
Community Substance Abuse Prevention
Individuals and community agencies working together as coalitions in Coos County and Northern Grafton County to develop and implement substance and tobacco use prevention activities. Funded by the DHHS Statewide Incentive Grant (SIG) and the NH Tobacco Prevention Program, the coalitions' activities include:
- tobacco prevention programs for youth and pregnant women;
- parent education for families with young children;
- support for Student Assistance Programs in local schools;
- assistance to schools in substance abuse prevention efforts;
- alternatives for youth through community action;
- tobacco prevention for you; and
- elimination of second-hand smoke.
Community Care Coordination
With funding from the Healthy New Hampshire Foundation and a grant from the US Department of Health and Human Services, HRSA Bureau of Primary Care, NCHC is addressing the needs of the uninsured and underserved population in Northern New Hampshire through:
- development of a Care Coordination model to facilitate patient access to coordinated health and social services to improve the patient's health status and reduce inappropriate utilization of services; and
- increased access to pharmacy and dental health services.
Community Health Initiative
In cooperation with the NH Department of Health and Human Services, the Community Health Institute and local municipalities, NCHC is developing a local public health coalition to ensure the capacity for provision of essential public health functions to the region.

Rural Transportation Initiative
With funding from the Endowment for Health and the Northern New Hampshire Foundation, NCHC is convening rural medical and public transportation providers and rural health care providers to:
- develop coordinated system of transportation in Northern New Hampshire; and
- increase access to medical, education, social and employment opportunities.
North Country Mobile Dental Services
North Country Mobile Dental Services (NCMDS) is an oral health access initiative developed by the North Country Oral Health Working Group with administrative support from the North Country Health Consortium. The project will provide access to dental services for north country residents who experience barriers to accessing dental care in traditional dental practices. The mobile model transports state of the art dental suites, customized to be easily portable and transported via truck, to provide dental clinic sites in community locations such as schools and hospitals in northern Grafton and Coos Counties.
- The project will support diagnostic, preventive and restorative services and minor oral surgery
- The first target population are Medicaid (NH Healthy Kids) eligible children who do not have a dental care home or have difficulty getting to the dental office because of transportation or other barriers.
- Funding is provided by grants from the NH Department of Health and Human Services Medicaid program, the Endowment for Health and North Country hospitals

National Rural/Frontier Women's Health Coordination Center
This North Country Health Consortium project works to identify, coordinate, and leverage a network of existing resources to provide a full range of health services to North Country women and their families. This initiative is sponsored by the U.S. Department of Health and Human Services
